STOP-WORK AUTHORITY NOTICE

A stop-work notice records that work was paused because of an unsafe condition — what the hazard was, who ordered the stop, the immediate actions taken, and the conditions that must be met before work resumes. Fill it in on this page and download an editable Word file to sign and keep. Any worker has the right to stop work they reasonably believe is unsafe.

This form documents a stop-work decision and the steps around it. It does not assess field conditions, decide whether work is safe to resume, or establish OSHA compliance. The decision to stop work and to resume it rests with the people on site and their competent person.
